چکیده

Introduction Large, segmental, osseous defects remain a major clinical problem. Common approaches to healing such defects include the use of autograft bone and the application of recombinant, human bone morphogenetic protein-2 (BMP-2) in the form of InfuseTM. Neither, however, is ideal. Supplies of autograft are limited and its harvest can be associated with considerable morbidity. BMP-2 is extremely expensive and its clinical performance in long bone healing is modest. Moreover, new analyses of data from the use of BMP-2 in spinal fusion suggest the potential for considerable, adverse side effects. The present study examined the potential for synergy between living bone particles and BMP-2 that, if present, could reduce the need for both autograft and BMP-2, thereby reducing costs and increasing safety. Bone particles were recovered with a device known as the “Reamer-Irrigator-Aspirator” (RIA) that recovers small osseous particles as it reams the intramedullary canal, and is increasingly used as a source of autograft.

Improved healing of large segmental defects in the rat femur by reverse dynamization in the presence of bone morphogenetic protein-2.

BACKGROUND Large segmental defects in bone do not heal well and present clinical challenges. This study investigated modulation of the mechanical environment as a means of improving bone healing in the presence of bone morphogenetic protein (BMP)-2. Although the influence of mechanical forces on the healing of fractures is well established, no previous studies, to our knowledge, have described ...

Vancomycin-bearing synthetic bone graft delivers rhBMP-2 and promotes healing of critical rat femoral segmental defects.

BACKGROUND Bone grafts simultaneously delivering therapeutic proteins and antibiotics may be valuable in orthopaedic trauma care. Previously, we developed a poly(2-hydroxyethyl methacrylate)-nanocrystalline hydroxyapatite (pHEMA-nHA) synthetic bone graft that, when preabsorbed with 400-ng rhBMP-2/7, facilitated the functional repair of critical-size rat femoral defects. Recently, we showed that...

Improving bone formation in a rat femur segmental defect by controlling bone morphogenetic protein-2 release.

Nonunion is a common complication in open fractures and other severe bone injuries. Recombinant human bone morphogenetic protein-2 (rhBMP-2) delivered on a collagen sponge enhances healing of fractures. However, the burst release of rhBMP-2 necessitates supra-physiological doses of rhBMP-2 to achieve a robust osteogenic effect, which introduces risk of ectopic bone formation and severe inflamma...

Healing of rat femoral segmental defect with bone morphogenetic protein-2: a dose response study.

OBJECTIVE Use of recombinant human bone morphogenetic protein-2 (rhBMP-2) is becoming a common clinical approach to enhance bone repair. There is little or no information in the literature on the dose of rhBMP-2 required for effective healing of critical-sized defects such as those associated with trauma. In this study, we used a segmental defect model to assess the dose response of rhBMP-2 usi...
